Preschool Program Information

We are now accepting applications for the 2017-2018 school year.  Once available spots are full, all registrations forms will be added to a wait list.

The following information will be needed to complete your child’s application:

  • The student’s legal first and last name, gender and date of birth.
  • Full address for the student and name, phone number and email of parent/guardian residing with the student.
  • If there are any students currently attending Ballard Community School District.
  • Copies of child’s birth certificate or other proof of student’s date of birth and social security card (optional) also need to be submitted with registration forms.

3-year-old, student's birthdate must fall on or between the dates of 9/16/2013-9/15/2014.

4-year-old, student's birthdate must fall on or between the dates of 9/16/2012-9/15/2013
